Patient Registration

New Patients

The Practice list is currently open and we are accepting eligible patients to register on our list,

To Register with the Practice we require two forms of identification, one of which is photo Identification (e.g. passport), you also need to provide proof of your current address, this could be a utility bill or a bank statement.
You will then need to complete the registration pack and GMS1 form which can be printed from the links below, these are also available from reception.

To register a child you will need to bring the child's birth certificate and The Personal Child Health Record (also known as PCHR or 'red book')

All patients should be aware that although they will have a named GP as their registered doctor they are actually registered with the Practice and can be seen by any of the GP's at the Practice. All Practitioners have access to your medical records and are able to provide a full range of consultations, medical advice and medication.

Patients may see a Practitioner of their choice, but you will have to wait longer to see a particular GP, and if you have an acute medical need you will have to see the Duty Doctor or the Nurse Practitioner. Please inform the Receptionist at the time of booking the appointment if you have a preference as to who you see.

Current Patients

Please notify the surgery of any change in circumstance, i.e. name, address, phone numbers etc.

If you are registered for patient online services you can sign in below and change your details electronically, you can also print our Change of Address form and return it to us or speak to one of our receptionists.
It is very important that we have all your correct details in case we need to contact you.
