Linden Medical Group is not a specialist travel clinic. However, some of our Practice Nurses have completed the necessary training to provide NHS travel vaccinations and general travel health advice.
To request travel advice or vaccinations, please contact the practice on 01536 481734 or submit a request via Anima. We will send you a travel assessment form to complete and return.
Important Information
Most travel vaccinations require a course of injections and need time to become effective. Wherever possible, please seek travel health advice 8–10 weeks before your departure date.
If you require a travel vaccination appointment, please complete and return the travel assessment form as soon as possible. A separate form must be completed for each traveller, including children.
Once we receive your completed form, one of our Practice Nurses will review the information and contact you to arrange an appointment if required.
Please note that we do not provide private travel vaccinations. If your travel plans require non-NHS vaccines, such as Yellow Fever, you should contact a specialist travel clinic or recognised travel vaccination centre.
