You can contact a doctor, nurse or other healthcare professional online using a website called Accurx.

Appointment requests

All requests for appointments need to go through the Accurx system, available from 6am to 10pm, Monday to Friday. A member of our clinical team will review the information provided and arrange a suitable appointment or signpost you to another service. The online form can be completed by family members or friends on behalf of a patient.

Alternatively, you can visit the surgery and complete a paper version of the form or call Linden Avenue Medical Centre at 01536 481734 where a care navigator will ask you a few questions to offer you the correct support.

Video consultations

We do not offer video consultations unless it is for a care home.

Phone advice

If you wish to speak to a GP or practice clinician we will try to accommodate your request, but we may ask you some questions to determine the urgency or nature of the call. A phone consultation may be offered to you with a GP; a GP will get back to you at their convenience.

Enhanced access

Different clinician appointments are available to 8pm Monday to Friday and 9am to 5pm on Saturday.

Online appointment booking

We have a facility that allows registered patients to book some appointments with doctors and nurses online. These include Long Term Condition appointments and Smear appointments.

First you must register for this service. This is a one-off process that involves you coming to the practice and requesting a username and password. Please bring photo identification with you for your own security.

Alternatively, if you are unable to attend the practice to register for online services, please call and speak to one of our receptionists after 10am and they will be able to register you over the phone by taking you through security questions. Once registered you will be able to book and cancel appointments in advance, as well as ordering any repeat medications, change your contact details and update your clinical record.

Cancelling or changing an appointment

Please make a note of your appointment time and contact us as soon as possible if you need to cancel. Although most patients do attend their appointments, every week a large number do not attend their booked appointments. The outcome of this severely hampers our ability to provide our patients with appointments.

If you have given us your up-to-date mobile number and have consented to receive SMS. You will receive a reminder of your appointment, if at that point you want to cancel your appointment, you can do so by replying to the message saying; ‘Cancel’. This will automatically cancel the appointment.

Late arrivals

Please note that if you are 10 minutes late for an appointment the doctor/nurse is likely to have moved on to the next patient. You may be asked to re-book your appointment this will be done according to the doctor’s discretion.

Please contact us as soon as possible if you know that you are going to be late, we will try to accommodate you where possible.

If you need help when we are closed

If you need medical help now, use NHS 111 online or call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

Home visits

Our doctors only provide home visits for patients who are housebound or residing in care homes. If you fall into one of these categories and feel you are too ill to attend the practice, please phone Linden Avenue Medical Centre at 01536 481734 to arrange a visit, ideally before 10am.

A doctor may contact you before attending. Visits are at the discretion of the doctor, and you will be asked by the receptionist to outline the problem.
