Complete a new patient registration form
If you live within the catchment area, please complete our new patient registration form.
When you register, its helpful to have your NHS number. You can use the NHS website to find number.
Paper forms are available from reception if you are unable to complete online form.
Gender Identity and Your Healthcare
If you are registering with the practice and your gender identity differs from the sex recorded at birth, please request a routine appointment with a GP. This appointment provides an opportunity to:
- Discuss your healthcare needs and any ongoing treatment or support.
- Confirm your preferred name, title and pronouns.
- Ensure your medical records are accurate and reflect your wishes where possible.
- Discuss consent for sharing relevant information within your healthcare team.
- Review any screening requirements and how these may relate to your biological sex and individual healthcare needs.
- Answer any questions you may have about accessing services at the practice.
Attendance at this appointment is voluntary, but we strongly encourage it to help us provide safe, appropriate and personalised care.
All discussions will be treated sensitively, confidentially and with respect.
Registering a looked after child or foster child
For looked after children or foster children, registration should be completed as soon as possible. Even if limited information is available, basic details can be provided initially, with updates made as more information becomes available.
