ADHD & Autism (ASD) referral pathway

Adults (18+) – How to request an assessment

1. Contact the practice to request a referral.

2. Reception will send you an Anima link with information and pass your request to the Secretaries.

3. You will receive an ADHD/ASD/Tourette’s questionnaire to complete.

4. A GP reviews your completed form:

  • If a referral is appropriate: The Secretary will contact you to confirm your preferred route (NHS, Right to Choose, or Private) and then complete the referral.
  • If a referral is not needed: The GP will contact you to discuss this or offer a follow‑up appointment.

Children and young people – How to request an assessment

1. Contact the practice.

2. Reception will send you an Anima message and pass your request to the Secretaries.

3. You will be sent a link to the School/Parent Pack to complete.

4. Once the completed pack is returned, the Secretary will confirm your preferred referral pathway and process the referral.

Referral Options & What They Mean

1. Local NHS Assessment – GP can support NHS prescriptions after diagnosis.

2. Right to Choose (RtC) Assessment – GP may prescribe if Shared Care criteria are met.

3. Private Assessment – GP cannot provide NHS prescriptions.
