New Patient Registration

To register a new patient you will need to live within our practice boundary.

If you are not registered with a doctor in the area at this time we will still be accepting new patient registrations, however please follow the new procedure detailed below;

Please download the registration pack, and registration form or phone 01536 527073 or 01536 527056 to request a pack to be sent in the post.

Once you have completed the registration forms, contact the practice to arrange a suitable time to attend the practice to complete your registration. You will need to bring two forms of ID with you (passport/driving licence and utility bill with your current address on, dated within the last three months).

When you attend to register, you will be greeted by a member of staff who will collect and check your registration forms and identification documents.

You will then be advised if any further information is required.
