To update details like your name, address, phone numbers or email address:
- use your NHS account (through the NHS website or NHS App)
- please submit an Admin Request via Anima.
Change of address
If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.
Changing your phone number or email address
You will need to show us identification before we can change your phone number or email address.
Changing your name
To change your name, please submit an Admin Request via Anima and attach a copy of one of the following documents:
- Marriage Certificate
- Civil Partnership Certificate
- Deed Poll
- Birth Certificate
If you are unable to upload your documents via Anima, please bring the original documentation to the surgery and a member of staff will be happy to assist you.
