Child new patient registration (0-16 years old)

Use this service to register a child up to 16 years old with the surgery.

You can use this service if you:

• want to register a child at the surgery
• live within our catchment area

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• your NHS number
• details of previous GP surgery
• basic health and medical information
