Please complete this questionnaire before your next review. This will help us decide whether you need to attend for a blood test and whether it is possible for you to have a telephone or video consultation with your diabetes nurse.
Try to complete as many of the questions as possible. As a result of COVID-19 we are trying to reduce the number of face-to-face reviews at the practice to support social distancing measures. However, the quality of care you receive will not be compromised.
Don’t forget to bring a urine sample if we ask you to attend for a blood test with our health care assistant or drop one in before your review with the nurse. This is a very important test to check your kidneys.
