Thank you for your enquiry about joining the practice – please find enclosed a Registration Pack.
Before you complete this pack please ensure your permanent address is within our practice registration area, we do not accept registrations outside of this boundary. This can be checked on our website Practice Boundary Page or you can ask a member of staff.
Please ensure you complete all the forms within this pack. These documents need to be returned with the following forms of identification and proof of address where possible.
Proof of Identity
- Passport
- Driving Licence
- Birth Certificate
- Marriage Certificate
Proof of Address
- Utility Bill
- Council Tax Statement
- Bank Statement
When you bring the completed form and supporting documents to reception, the team will check the forms are correctly filled out and will take a copy of your identification.
The registration process should take between 5 and 7 days, but please appreciate this may take longer during busy periods. If there are any problems with this, our registration team will be in touch.
