Sick (fit) notes

If You’ve Been Off Sick for More Than 7 Days

To request a sick note (also known as a fit note), please use the Accurx service. According to NHS guidelines, you will need a fit note if you have been unwell and unable to work for more than 7 consecutive days, including weekends and bank holidays. Fit notes can be issued by various healthcare professionals, including doctors, nurses, pharmacists, physiotherapists, and occupational therapists.

If You’ve Been Off Sick for 7 Days or Less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence. Instead, they can request that you confirm your illness through self-certification. This involves filling out a form yourself when you return to work. Self-certification is a straightforward process that helps streamline the handling of short-term sickness absences.

How to Self-Certify

  • Fill Out a Form: You can complete a self-certification form, which is often provided by your employer or available on the GOV.UK website.
  • Provide Details: Include the dates of your sickness and a brief description of your illness.
  • Submit to Employer: Return the completed form to your employer upon your return to work.
