Linden Medical Group is seeking a motivated Care Coordinator to join our friendly and dedicated team.
As a Care Coordinator, you will play a key role in supporting patients, particularly those with complex health needs, by helping them navigate health and care services, coordinating appointments and referrals, and ensuring they receive the support they need to achieve the best possible outcomes.
Main Responsibilities
- Coordinate patient care across health and social care services.
- Support patients to access appropriate services and community resources.
- Work closely with GPs, nurses, care homes, community teams, and external agencies.
- Monitor and follow up patient care plans and actions.
- Assist in identifying patients who may benefit from additional support and proactive care.
- Maintain accurate patient records and documentation.
- Provide excellent customer service when dealing with patients and healthcare professionals.
About You
The ideal candidate will:
- Have excellent communication and organisational skills.
- Be confident working with a wide range of healthcare professionals and agencies.
- Demonstrate empathy, compassion, and a patient-centred approach.
- Be able to manage a varied workload and prioritise effectively.
- Have good IT skills and experience of working with clinical systems (desirable).
- Have previous experience within healthcare, social care, or a customer-focused environment (desirable).
